Do it all on the phone itself — the card has to end up there. You need an email address. Nothing else is required except your name and a password.

Email, full name, a password of at least 6 characters, and a 6-digit PIN typed twice. The PIN lets a paramedic read your medical details — it is printed on your card, so you never have to remember it.
Now fill in what matters to you: blood type, allergies, daily medication, next of kin. Then tap “Save changes”.
Your card appears as a picture. Press and hold on it until the menu appears, and choose “Add to Photos”.
Now your name, blood type and QR code can be read without unlocking the phone — also by a paramedic who does not know you.
Your email and the password you chose. Then tap point 1 — “Edit information”. The fields are locked until you do.
Enter the country and the address where you are staying now. It appears on your card under your name. Leave them empty and your home address is shown. Finish with “Save changes”.
